Successful treatment of Candida tropicalis osteomyelitis with Micafungin in a leukemia patient

چکیده
The incidence of invasive candidal infection is increasing. The widespread use of central intravascular catheters, invasive procedures, immunosuppression, diabetes mellitus, total parenteral nutrition, broad-spectrum antibiotics and drugs abuse predispose patients to the infection. Eye, kidney, liver, skin, cardiac and brain infections are the most common site of invasive candidiasis while vertebral osteomyelitis is rare. Most Candida vertebral osteomyelitis is caused by Candida albicans. Less common organisms include C. tropicalis, C. glabrata and C. parapsilosis [1]. Both amphotericin B and the azoles have a role in the treatment, but treatment failure with either agent had been reported [2,3]. Micafungin belongs to echinocandins, and interferes with the synthesis of fungal cell wall. We report a Candida tropicalis infection manifested with osteomyelitis following a previous candidemia in a leukemia patient who was successfully treated by Micafungin.

Clinical characteristics of the first cases of invasive candidiasis in China due to pan-echinocandin-resistant Candida tropicalis and Candida glabrata isolates with delineation of their resistance mechanisms
Echinocandin antifungal agents have become the first-line therapy for invasive candidiasis (IC) in many countries. Despite their increasing use, resistance to this class of drug is, overall, still uncommon. Here, we report two patients from the People's Republic of China with IC, one with infection caused by pan-echinocandin-resistant Candida tropicalis and the other by pan-echinocandin-resista...

Sir, Candida species is the fourth leading cause of nosocomial bloodstream infection in the USA. The incidence and mortality of invasive candidiasis (IC) remain high despite new antifungal agents. Although Candida albicans remains the most common isolated species causing IC, the incidence of IC caused by non-albicans species is increasing. Caspofungin, an echinocandin, is approved for treatment...

Candida tropicalis, a species closely related to Candida albicans, is an emerging fungal pathogen associated with high mortality rates of 40 to 70%. Like C. albicans and Candida dubliniensis, C. tropicalis is able to form germ tubes, pseudohyphae, and hyphae, but the genes involved in hyphal growth machinery and virulence remain unclear in C. tropicalis. Recently, echinocandin- and azole-resist...

We report a case of spondylodiscitis concurrent with infectious aortic aneurysm caused by Candida tropicalis in a 79-year-old immunocompromised man. The patient underwent percutaneous drainage of the abscess and endoluminal exclusion of the aneurysm using a bifurcated stent graft. Micafungin was administered intravenously for 8 weeks, followed by a prolonged course of oral fluconazole. At the o...

Due to unacceptably high interlaboratory variation in caspofungin MIC values, we evaluated the use of micafungin as a surrogate marker to predict the susceptibility of Candida spp. to caspofungin using reference methods and species-specific interpretive criteria. The MIC results for 3,764 strains of Candida (eight species), including 73 strains with fks mutations, were used. Caspofungin MIC val...
